Shockwave Therapy for Neck Pain

Shockwave Therapy for Neck Pain

Neck pain is extremely common and can make everyday activities difficult, from working at a computer and driving to exercising or getting a good night’s sleep.

For many patients, neck pain is related to tight or painful muscles, trigger points, and irritation of the soft tissues surrounding the neck and upper back.

Shockwave therapy for neck pain is a non-invasive treatment that uses acoustic pressure waves to stimulate a biological response in painful muscles and soft tissues. Research suggests that shockwave therapy may help reduce pain and improve function in certain patients with chronic neck pain, particularly when myofascial pain and muscle trigger points are involved.

The first step is determining what is causing your neck pain and whether shockwave therapy is appropriate.

What Is Shockwave Therapy?

Extracorporeal Shockwave Therapy (ESWT) uses acoustic pressure waves that travel through the skin into a targeted painful area.

Despite the name, there is no electrical shock.

The acoustic energy is believed to produce several effects that may help with chronic muscle and soft-tissue pain, including:

  • Increased local blood flow
  • Changes in pain signaling
  • Stimulation of tissue repair
  • Reduced muscle stiffness
  • Treatment of painful trigger points
  • Improved movement and function

Shockwave therapy is performed in the office and does not require an incision or injection.

Common Types of Neck Pain Shockwave Therapy May Help

Myofascial Neck Pain

Myofascial pain is one of the most common causes of chronic neck and upper-back discomfort.

It develops when muscles and the connective tissues surrounding them become painful and sensitive.

Patients may experience:

  • Aching neck pain
  • Tightness or stiffness
  • Pain between the neck and shoulder
  • Tender areas in the muscles
  • Pain after working at a computer
  • Pain that becomes worse after prolonged sitting
  • Limited neck movement

Shockwave therapy can target the painful muscles and surrounding tissues to help reduce pain and muscle tightness.

Upper Trapezius Pain and Trigger Points

The upper trapezius is a large muscle that extends from the neck to the shoulder.

Trigger points within the trapezius can cause significant neck and shoulder pain. These areas may feel like painful knots and can sometimes cause pain that travels toward the shoulder or head.

Research on shockwave therapy for neck pain has focused heavily on upper trapezius myofascial pain and trigger points.

Shockwave therapy may help decrease pain sensitivity, reduce muscle stiffness, and improve movement in selected patients.

Chronic Neck Pain

People who work at computers, spend long periods looking down at phones, drive for extended periods, or perform repetitive work can develop persistent neck muscle pain.

Chronic neck pain is often caused by several factors rather than one single abnormality.

Shockwave therapy may be combined with:

  • Exercise
  • Stretching
  • Posture improvement
  • Physical therapy
  • Strengthening
  • Ergonomic changes

The goal is to reduce pain while also addressing the factors that may be contributing to the problem.

Cervical Arthritis and Spondylosis

Cervical spondylosis, sometimes called arthritis of the neck, refers to age-related changes involving the joints, discs, and other structures of the cervical spine.

Some patients with cervical arthritis also have significant muscle pain, stiffness, and painful soft tissues surrounding the neck. Treating these areas may help improve overall pain and movement.

There is also emerging research looking specifically at focused shockwave therapy for cervical spondylosis, with some studies showing improvements in pain, neck function, and range of motion.

What Does the Research Say About Shockwave Therapy for Neck Pain?

Several clinical studies and systematic reviews have evaluated shockwave therapy for chronic neck and upper trapezius pain.

A 2024 systematic review and meta-analysis of 27 clinical trials evaluated shockwave therapy for myofascial pain. Researchers found that shockwave therapy significantly reduced pain and improved function compared with control treatments and therapeutic ultrasound. The researchers noted that results varied between studies.

More recently, a 2025 randomized controlled trial studied focused shockwave therapy in office workers with chronic upper trapezius myofascial pain. Researchers found reductions in muscle stiffness and pain following treatment.

Overall, the research suggests that shockwave therapy may be a useful non-surgical treatment for certain forms of chronic neck pain, especially when painful muscles, trigger points, and myofascial pain are major contributors.

Why Choose Shockwave Therapy for Neck Pain?

Non-Invasive

Shockwave therapy does not require surgery or an incision.

No Injection Required

There are no medications or needles injected during shockwave treatment.

Minimal Downtime

Most patients can return to normal daily activities shortly after treatment.

Some temporary soreness or tenderness may occur in the treated muscles.

Targets Painful Muscles and Trigger Points

Shockwave therapy allows us to target painful muscles and soft tissues that may be contributing to neck pain.

This can be particularly useful when the pain is coming from the upper trapezius or other muscles surrounding the neck and shoulder.

Can Be Combined With Rehabilitation

Shockwave therapy is often most useful as part of a complete treatment plan.

Depending on the cause of the neck pain, treatment may also include:

  • Neck and shoulder strengthening
  • Stretching
  • Physical therapy
  • Posture correction
  • Ergonomic changes
  • Activity modification

The goal is not simply to temporarily cover up the pain but to improve how the neck and surrounding muscles function.

What Does Neck Shockwave Therapy Feel Like?

During treatment, patients usually feel a rapid tapping or pulsing sensation over the targeted muscle.

Painful trigger points may feel more sensitive during treatment.

The intensity can usually be adjusted based on the patient’s comfort and the condition being treated.

Treatment is performed in the office and typically does not require anesthesia.

How Many Shockwave Treatments Are Needed for Neck Pain?

The exact number of treatments depends on the cause of the neck pain and the patient’s response.

At our office, many patients receive approximately 6 shockwave therapy sessions over 4–6 weeks.

Some patients notice improvement during the treatment series, while others continue improving over the following several weeks.

Because shockwave therapy is intended to stimulate a biological response, improvement may be gradual rather than immediate.

Is Shockwave Therapy Right for Your Neck Pain?

You may be a candidate for shockwave therapy if you have:

  • Chronic neck pain
  • Myofascial neck pain
  • Upper trapezius pain
  • Painful muscle trigger points
  • Neck and shoulder muscle tightness
  • Certain cases of cervical arthritis with surrounding muscular pain
  • Neck pain that has not improved enough with conservative treatment

However, not all neck pain should be treated with shockwave therapy.

A herniated disc, pinched nerve, spinal stenosis, fracture, spinal cord compression, or other cervical spine condition may require a different treatment approach.

The most important step is identifying the root cause of your neck pain.
